This module aims to develop students’ ability to assess, treat, refer and discharge patients with long-term conditions. The module focuses on the improvement of students’ knowledge of clinical referral pathways and decision-making skills. This module will also cover the decision making process in relation to the prevention of inappropriate hospital attendance.WHY CHOOSE THIS COURSE?

On successful completion of this module you should be able to:
- Assess the clinical decision making process.
- Understand the referral pathways and support available for people with long-term conditions.
- Examine and understand the requirements for urgent intervention or transfer to appropriate level of care when needed.
- Understand the psychological and physiological impact of a long-term condition on the patient and their family.
- Note the influences on the care provision of people with long-term conditions.
This module will be delivered online during the Spring term, through discussion forums and a range of interactive online activities.
Induction will take place on the first day of each module (excluding Mentoring).
